I authorize any former employer, person, school, firm, or corporation listed on this application — including this company — to provide information regarding my employment history. I agree to release from liability and hold harmless all persons who provide truthful information within their knowledge or records. I understand this is a preliminary application and not an employment contract. If employed, I understand that my employment will be completely voluntary and may be terminated at any time by either myself or the company. I further understand that no one other than the company president has the authority to enter into any employment agreement contrary to the foregoing. I agree to take a job-related physical examination and/or drug or alcohol test if requested as a condition of employment. I also agree to comply with all company rules and policies as a condition of employment. In the event the company advances me money or other things of value, I agree to repay the company in full.
